In the Add Shipping Address popup, the State and City dropdowns display selectable values even when no Country has been selected. State and City options should be dependent on the selected Country.

When a user selects a Country, State, and City in the Add Shipping Address popup and then clicks Cancel, reopening the popup retains the previously selected State and City values even when no Country is selected. The form is not reset properly.
